Because Skin Works School of Advanced Skin Care is accredited by the Council of Occupational Education (COE) and have been approved by the Department of Education (DOE), we are approved to offer Title IV financial aid to those who qualify. Skin Works School offers the following Title IV programs:

1. Pell Grant

2. Subsidized and Unsubsidized Stafford Loana

3. Parental PLUS loans
(for dependent students)

 

Any student interested in attending Skin Works and receiving Title IV Financial Aid to cover tuition costs must completed an application for financial aid.

The application can be completed online at http://www.fafsa.ed.gov or in-person by making a financial aid appointment by calling (801)530-0001.

Paper applications are available to be mailed to you as well.

If you have an appointment with the financial aid office, we will be able to answer any questions you may have on the application. Once the application is received by our offices, we will process your Free Application for Federal Student Aid (FAFSA) within 3-business days. The application will be sent to the Central Processing System (CPS) and you should receive a verification of your application by way of e-mail or street address.

Verification

If there are any discrepancies in processing, we will send you a letter or call within 3-business days of receiving the information. If a student is selected for verification, Skin Works gives students a deadline of 7-business days (in-state) or 10-business days (out-of-state) to complete verification. Failure to complete verification within 30 days could result in owing cash for the program. Skin Works is dedicated to working with its students if documents are not provided within the specified time-frame.

For your convenience, if there are any changes to be addressed/made to your Student Aid Report (SAR) please notify the school. We will process necessary information in-house.

If the result of verification changes the student award, a new award letter will be sent by mail or in-person, whichever is most convenient.

Once enrolled into the college, please note that Title IV money cannot be disbursed until all discrepancies/verification items are addressed, we have a valid output document (SAR/ISIR) and an award letter is signed.

If we suspect that a student, employee or other individual has misreported information or altered documentation to increase student aid eligibility or to fraudulently obtain federal funds, we will report any suspicions and provide any evidence to the Office of Inspector General at 1-800-MIS-USED or on-line at: http://www.ed.gov/ofices/OIG.


Skin Works School Institutional Refund Policy – Return of Title IV Funds Policy

As part of the Higher Education Amendments of 1998, Congress passed new provisions governing what must happen to Federal financial assistance if a student withdraws from school in any program length. If a student withdraws from Skin Works School, then the school or the student may be required to return some of the federal aid awarded to the student. The student may also be eligible for a refund of a portion of the tuition and fees paid to SKIN WORKS SCHOOL for the length of the program.


Institutional Refund Policy

A student shall be refunded, within 60 days of his/her last day of attendance, a percentage of all tuition paid over and above a nonrefundable registration fee of $100.00. Refunds, when due, shall be made with 30 days of the last day of attendance if written notification of withdrawal has been provided to the institution by the student. The balance due will be calculated using the following schedule.

SkinWorks Return of Title IV Federal Aid Policy

Federal regulations assume Federal financial aid awards are earned directly in proportion to the number of days of the program attended until the withdrawal date. If withdrawn from school during a portion of the total hours the school must calculate, according to a specific formula, the portion of the total scheduled financial assistance earned and are therefore entitled to receive up to the withdrawal date. If SKIN WORKS SCHOOL receives more assistance than earned, the unearned excess funds must be returned to the Department of Education. On the other hand, if SKIN WORKS SCHOOL receives less assistance than earned, you may be able to receive those additional funds.

The portion of earned Federal student aid is calculated on a percentage basis by comparing the total number of days in the program length to the number of days completed. For example, if 30% of the program length is completed, 30% of the Federal aid is earned. This means that 70% of the scheduled award for this period remain unearned and must be returned to the Department of Education.

Once 60% or more of the program length is completed, all (100%) of your assistance is considered earned. If you withdraw from SKIN WORKS SCHOOL before completing 60% of the program length, you may have to repay any unearned financial aid funds that were already disbursed to you.

If excess funds are received based on this calculation, SKIN WORKS SCHOOL must return a portion of the excess equal to the lesser of:

  The student’s institutional charges multiplied by the unearned percentage of funds, or

  The entire amount of the excess funds.


If SKIN WORKS SCHOOL is not required to return all the excess funds, the student must return the remaining amount. The order that SKIN WORKS SCHOOL and the student must return these funds is as follows:

   Unsubsidized Federal Stafford Loan

   Subsidized Federal Stafford Loan

   Federal PLUS Loan

   Federal Pell Grant

   Other Programs

   Scholarship program

If you are required to repay loan funds, this is done in accordance with the terms of your loan promissory note. If you must repay any grant funds, the law states that you are required to repay only 50% of the grant assistance was calculated to repay. Any grant amount that a student must repay is considered a grant overpayment and therefore must be repaid to SKIN WORKS SCHOOL or the Department of Education within 45 days or it will be reported to the Department of Education.

Skin Works is nationally accredited with the Council On Occupational Education (COE).



Provisions For Makeup Work


Open lab days are available from 10:00 AM to 4:00 PM on Saturday except holidays

Students need to make arrangements with instructor to makeup class.


Calendar and Holidays


Skin Works is closed on all Bank Holidays, Sundays and other designated Holidays in the registration materials given at enrollment.


Student Council


Each class has 1-2 class representatives to bring student suggestions and student grievances to the student council held each month. A faculty and administration will also be in attendance to help come to an acceptable resolution or plan.

Classroom representatives are elected at the beginning of each new class.



Student Records


Permanent records are kept for all students including the following:

  Grades

  Attendance

  Transcripts

  Diploma’s

  Grievances, probation forms



Honor Students


Students that receive above 90% on their classwork and attendance will receive the status as an Honor Student on their SAP reports and on their final diploma.

Instructors will also select one student each month to be honored as the Student of the Month.


Dismissal Policy

A student will be dismissed for any of the following:

  Failure to abide by any policy contained in this Catalog/Student Handbook.

   Cheating, stealing, or vandalizing.

   Unprofessional conduct.
